As a student in Elkhart Cyber School I agree to the following terms and conditions:

  • I voluntarily request participation in Elkhart Cyber School and have read and understand the terms of this agreement.
  • I agree to participate in and complete all courses listed on my Individualized Study Plan.
  • I will complete all assignments given to me during the time that this agreement is in effect by the due date stated on each assignment. 
  • I will contact each of my instructors a minimum of once per week to report my progress. 
  • I agree to abide by the Academic Honor Code: I will neither give nor receive unauthorized aid, nor will I tolerate an environment which condones the use of unauthorized aid.  Failure to abide by this code will result in disciplinary action by ECS instructors and/or ECS administration.  Discipline may result in dismissal from ECS.
  • I agree that all coursework completed for ECS will be my authentic work and understand that any violation of academic integrity will result in disciplinary action by ECS instructors and/or ECS administration.  Discipline may result in dismissal from ECS.
  • I agree to complete the Daily Activity Log each day and to submit it to the ECS office at the close of each quarter.
  • I understand that I must participate in the two mandatory work days - September 15th & 22nd - a fall testing session, and any required state testing in the spring. 
  • I agree to abide by all other policies and requirements outlined in the ECS Student Handbook.
  • I understand that failure to meet any of these minimum requirements will result in my placement on the Intervention Plan as described in the ECS Student Handbook.

 

Computer and Network Usage

  • I understand and will abide by ECS guidelines and conditions for the use of ECS equipment and access to the Internet.  I further understand that any violation of ECS guidelines is unethical and may constitute a criminal offense.  Should I commit any violation, my access privileges may be revoked and all ECS materials and supplies will be forfeited back to ECS.  School disciplinary action and/or appropriate legal action may be taken.
  • I agree not to hold ECS nor any of its employees nor any of the institutions nor networks providing access to ECS responsible for the performance of the system or the content of any material accessed through it or damage to personal equipment caused by computer networking through ECS.
  • I understand that only ECS personnel, or a qualified person designated by them, are authorized to make repairs or add additional equipment to ECS computers.  If I violate this rule, I will be held responsible for any damages that occur.
  • I understand that all computer boxes and packing materials remain the property of ECS.  I agree to keep them in their original condition for future use.
  • I agree to be held financially accountable for any damage, deliberate or accidental, that occurs to ECS equipment checked out to me. 
  • I agree to abide by all terms and conditions mentioned in the computer and textbook lease.
  • I agree to return the computer and all materials within 30 days of leaving ECS.

My signature below indicates that I voluntarily participated in the establishment of this agreement and that I understand and accept my responsibilities in relation to this agreement.
